The free NICHE Program will help you to learn more about your own health and condition(s).
Our locally based health professional will assist you in gaining more confidence in managing your own health and in making positive changes. The program will work with your GP and other health professionals to ensure everyone is up to date on your health, with you at the centre.
At the end of the program you will able to continue on with managing your health with the help of your health care team.
